1. PLAYER REGISTRATION AND CONSENT FORM: Please ensure that the Player Details, Parent/Guardian Details, Medical Details and Payment Details are completed fully and all Consents signed

2. All players will be issued with a Club number. The Club numbers need to be retained by you and used for all correspondence with the club and to enable WO Sports to allocate payments correctly

3. LEAGUE REGISTRATION FORM: Please ensure that this is completed neatly and legibly and signed as appropriate.

4. Handbook will be given out to make yourself aware of WO Sports rules and key information, our respect codes for parents and players and our introduction to the club for players and parents/guardians.

5. By agreeing to register your child/ren with WO Sports, You are agreeing for us to manage the development of your child/ren. This means WO Sports reserve the right to move your child/ren with two week’s notice to another team within WO Sports where your child/ren abilities are suited. This decision will be made by the coaches of the two teams in question and will involve input from the head coach at WO Sports.

WO Sports must have a photocopy of the player’s passport or birth certificate must also be provided upon request.

Registration payment of £160.00 will be paid to WO Sports by Direct Debit.

Players will only be registered with a League when the form is completed in full, the Registration Fee payment has cleared, a Club number issued and any back subs are cleared.

A) All times whilst representing WO Sports to conduct yourself in a manner which enhances the sporting reputation of WO Sports and its Image of well-behaved and disciplined teams.

B) All monies are payable by ‘Direct Debit. Failure to pay on a regular basis will result in suspension from the squad.

C) Under no circumstances will the club tolerate the use of foul and abusive language, or behaviour, towards others, be it aimed at fellow squad members, your coach, opposition players, officials, or parents.

D) Firm disciplinary action will be taken against you if any of the above are proven.

E) At all times, respect and look after all training equipment and football kits. Your team coach gives up their time for the sole purpose of providing you with training and matches. Listen to them and respect their views.

F) All members want to play in the side but regretfully this is not always possible. If you are not selected support your teammates.

G) When playing or training with WO Sports you are registered as a WO Sports player. Only if selected for your school side, or a representative side for training or matches, are you permitted to train or play for any other team. If another club approaches you, you must report this to your parent/guardian and WO Sports.

WO Sports spectators

We would respectfully advise parents, relatives and friends of members playing for WO Sports that all decisions relating to playing, policy/selection and discipline are the responsibility of the team coach.

The team coach is also responsible for all WO Sports spectators when attending his side’s games. Please remember this when attending a match as any acts of misconduct may result in the club being fined. Parents etc are encouraged to support their team but also show respect to the opposition. Communication with players is the responsibility of team representatives during games, coaching by parents will not be tolerated.

2025-2026 PAYMENT FORM

WO Sports has agreed that fees for the coming season are as follows

• REGISTRATION FEE: £160.00 per player for U6s-U15s. This is a one-off payment when the player registers with WO Sports and you’re entitled with 2 kits(Home and Away). There is 10% discount for siblings.

• MONTHLY FEE: £65.00 per player for U10s-U15s. This covers one night training session a week on Thursday from 6.00pm-7.30pm at Feel Good Too(Formerly ive Farm) Leyton, E10 5UE for training and official matches (League, Cup and Friendlies) during the period 7th September 2025 to 18th July 2026. U6s-U9s (Wednesdays at Feel Good Too 6pm-7.30pm).

• METHOD OF PAYMENT: The preferred method of payment will be by Direct Debit

NOTE:

A) A player will not be registered with the Club or a League unless the Registration Fee and any subs owed have been cleared.

B) The Annual Fee is fixed and is not related to the number of training sessions/matches actually attended by a player in the period.

C) In the event of long-term injury or personal circumstance which may result in a player not being able to attend training/and or matches for a period greater than 4 weeks, consideration will be given to a reduction in annual fee.

D) Any player who has not paid a monthly fee without a valid reason will be suspended from training/matches until the debt is cleared.

E) Training kit must be purchased and worn at all WO Sports training sessions.

F) Club Tracksuit must be worn to games and training sessions.

G) Should in case of pandemic, monthly payment shall be stopped upon the club’s instructions.

H) Adequate insurances had been taken to ensure WO Sports players and coaches are protected to resume training and participate in competitive games.

  • This Guarantee is offered by all banks and building societies that accept instruction to pay Direct Debits.
  • if there are any changes to the amount,date or frequency of your Direct Debit takepayments Limited will notify you three working days in advance of your account being debited or as otherwise agreed. If you request takepayment Limited to collet a payment,confirmation of the amount and date will be given to you at the time of the request.
  • If an error is made in the payment of your Direct Debit, by takepayments Limited or your bank or building society you are entitled to a full and immediate refund of the amount paid from your bank or building society.
  • you can cancel a Direct Debit at any time by simply contacting your bank or building society. written confirmation may be required. Please also notify us.

Sports coaches or club representatives, so please do not put them in a position to refuse. Failure to set up and maintain payment by Direct Debit, in line with the details above will result in your child not being signed by WO Sports.

As a FA chartered standard club, All of our coaches are a minimum FA Level 1 qualified, several at Level 2 and UEFA B Level 3, in addition some are qualified to FA Youth Module 2 in age appropriate training and some in Education at University and Sports Colleges and some are in full time employment in Schools and Leisure Centres. All coaches are DBS checked, have attended FA Safeguarding Children Course, Child Protection and are qualified First Aiders.

Parental and Player Consent: By returning this completed form I agree to the child named taking part in the normal activities of WO Sports.. I have read the Code of Conduct for both players and parents/guardians/ spectators, the Clubs Disciplinary Procedures and Guidance and agree to abide by those whilst in the care of the club and I understand that any serious or continued breach of these codes may result in my child being expelled from the club.
